Responsibilities
FOR INTEGRATED CASE MANAGEMENT, AND RESIDENTIAL: Provides community-based mental health case management. Degree and scope of service intervention vary based on program as well as the changing needs of the individual. FOR ADOLESCENT PARTIAL CARE, ADULT PARTIAL CARE, AND ACUTE PARTIAL HOSPITAL: Responsible for providing direct care services including individual and group counseling, support and other activities; participating in comprehensive assessment, development of treatment and discharge planning; maintaining clinical documentation, participating in and facilitating family and collateral meetings, team meetings and case conferences to ensure continuity of care.#LI-NH2
Qualifications
EDUCATION, EXPERIENCE, TRAINING EDUCATION: 1. Masters degree from an accredited college or university in psychology, social work or rehabilitation. 2. Bachelor’s degree candidates currently enrolled in a Graduate Program and with additional experience will be considered on a case by case basis. 3. Two years mental health experience. 4. Bachelors’ level candidates will need 3 years relevant experience with at least 1 year in a mental health case management position. 5. Comprehensive understanding of mental health services and role of case managers in mental health services. 6. Valid New Jersey Driver’s License.Pay Transparency
