What You'll Be Doing
The Lead Social Worker is responsible for assessing, counseling, intervening, implementing and educating the patient, family/significant others to facilitate movement throughout the continuum of care.
What We're Looking For
Education: Bachelor’s degree
Experience: 2 –4 years related Experience
Skills: NJ Licensed Social Worker. Ability to communicate clearly in English, both orally and in writing. Knowledge of the discharge planning process. Knowledge of Medicare/Medicaid Regulations; community resources. Knwolege of the Joint Commission guidelines (continuation of care).
