Responsibilities
Under the direction and supervision of the Clinical Manager, the RN Assessment Coordinator is responsible for conducting and coordinating the completion of the resident comprehensive assessment which includes the Minimum Data Set (MDS), and for tracking, and transmitting all data including the Resource Utilization Groups (RUGs) to the Department of Health and Human Services. The process includes the coordination of appropriate participating health professionals (interdisciplinary team) for the purpose of conducting initial and periodical comprehensive, accurate assessments of each resident to plan care that allows the resident to reach his/her highest practicable level of physical, mental and psychosocial functioning. This position will provide coverage in the absence of the Clinical Manager.
