Responsible for assessing inpatients psycho-social history, assisting the clinical team in developing a treatment plan, participates in discharge planning process and coordinates follow-up care for patients. Acts as a liaison for family, friends, and therapists and outside agencies. Provides family therapy and education. Provide oversight to coordination of care with social workers within the department. Responsible for training and mentoring hospital staff. Assists leadership in identifying, developing and implementing process improvement initiatives
Job Requirements
In addition to bringing humankindness to the workplace each day, qualified candidates will need the following:
Master's degree in Social Work
Two years of psychiatric or mental health experience required.
Previous leadership experience preferred.
Demonstrates clinical and ethical professionalism, conflict resolution and critical thinking skills.
- Licensed Social Worker or Counselor Requires
Current State of employment License as a Clinical Social Worker (LCSW), - Professional Counselor (LPC), or
- Licensed Marriage and Family Therapist (LMFT)
BLS Required
Sedentary work - prolonged periods of sitting and exert up to 10 lbs. force occasionally
