Overview
We are seeking an experienced and licensed Registered Nurse (RN) in Texas to complete Comprehensive Nursing Assessments (CNA) in compliance with Texas Health and Human Services (HHSC) requirements for individuals receiving community-based services. The RN will conduct in-home assessments, document health needs, and develop service plans that promote the health, safety, and independence of individuals served.
Responsibilities
- Conduct Texas Comprehensive Nursing Assessments (CNA) in accordance with HHSC guidelines.
- Evaluate the individual’s physical, medical, and psychosocial status through observation, interview, and review of medical records.
- Maintain accurate, timely, and complete documentation of assessments and follow-ups.
- Conduct reassessments as required (annually or upon significant changes in condition).
- Maintain confidentiality and adhere to HIPAA and agency policies.
Qualifications
- Current, unencumbered Texas RN license (required).
- Minimum of 2 years clinical nursing experience; community-based, long-term care, or home health experience preferred.
- Strong knowledge of Texas Comprehensive Nursing Assessment requirements and HHSC documentation standards.
- Must be willing to obtain professional liability insurance.
- Ability to work independently, manage time efficiently, and meet strict deadlines.
- Excellent communication, clinical judgment, and interpersonal skills.
- Must have reliable transportation and willingness to travel to client homes.
