Job Summary
Position title: Care Coordinator
Job Summary: The Care Coordinator is responsible for building and maintaining strong relationships with patients, conducting in-person assessments, and developing individualized care plans in coordination with healthcare providers and caregivers. The ideal candidate will serve as a trusted advocate and central point of contact for patients, helping them navigate the healthcare system and access the resources they need to support their health and well-being.
Responsibilities
Build and maintain trusted relationships with patients through ongoing communication and support
Conduct face-to-face assessments to evaluate patients’ medical, social, and emotional needs
Develop personalized care plans in collaboration with providers, caregivers, and the care team
Coordinate and connect patients with appropriate community resources to address social determinants of health
Promote timely access to care, including preventive services and follow-up appointments
Facilitate referrals to primary care and specialty providers as needed
Assist with scheduling appointments and arranging transportation services
Serve as the main point of contact and advocate for patients, caregivers, and the broader care team
Educate patients on available health services and community support programs
Maintain accurate and up-to-date documentation in accordance with organizational and regulatory standards
Ensure high-quality, patient-centered care that improves outcomes and patient satisfaction
Qualifications
Bachelor’s degree in healthcare, social work, public health, or a related field preferred
Associate’s degree accepted with relevant experience in healthcare or social services
Minimum 2 years of experience in a healthcare setting
Prior experience in case management or care coordination is preferred
Strong interpersonal and communication skills
Ability to work independently and collaboratively within a care team
Proficient in documentation and electronic health records (EHR) systems
Knowledge of community health resources and patient advocacy
