Overview
Under the direction of the Clinical Supervisor or designee is responsible for providing social work services in assisting in the resolution of social and emotional problems through appropriate psychosocial assessment and discharge planning for those referred patients on an inpatient medical, surgical, or pediatric unit, or ambulatory care setting.
Responsibilities
- Establishes and maintains positive relationships with patients, visitors and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of Bronx-Lebanon Hospital Center and its employees
-Carry an active caseload of patient/families as stipulated by departmental policies.
- Provide social work services to patients/families, including bio psychosocial/cultural assessment and treatment; crisis intervention, short or long term, as indicated and to include modalities as individuals, family, etc.
- Collaborate with inter-disciplinary team in providing patient care.
- Provide reports, record documentation and statistics as required.
- May serve as liaison within the department, hospital and to community agencies.
- May be delegated responsibility for in-service training teaching social work students, etc. Represent social work department in job related community meetings and conferences.
- Perform other activities as shall be deemed appropriate in assuring services to patients and the functioning of the Social Work Program.
Qualifications
- Sequence of study or experience in clinical practice in accordance with current New State standards for licensure and previous experience in the appropriate inpatient or outpatient setting as indicated.
- New York State Licensed Social Worker (Legally Required) (Policy Requirement)
- Master's in Social Work (Required)
- Computer literacy (Required)
