Job Summary
The Associate Director for Children's Services will oversee the planning, implementation, and evaluation of comprehensive health services tailored to children and their families within the Health Home framework. This role requires a dynamic leader who is passionate about improving health outcomes for children, promoting family engagement, and ensuring the delivery of high-quality, coordinated care
Responsibilities
- Program Development and Management
- Develop, implement, and manage programs and services for children and families within the Health Home model
- Ensure that services are culturally competent, family-centered, and aligned with best practices and regulatory requirements
- Monitor program performance and outcomes, making data-driven decisions to enhance service delivery
- Lead continuous quality improvement initiatives to optimize care coordination and patient outcomes
Leadership and Supervision
- Supervise and support a multidisciplinary team of professionals, including case managers, social workers, and support staff
- Foster a collaborative and inclusive team environment that encourages professional growth and development
- Conduct regular team meetings, performance evaluations, and provide ongoing training and support
- Mentor staff in evidence-based practices and Health Home service delivery standards
Collaboration and Community Engagement
- Build and maintain partnerships with community organizations, schools, healthcare providers, and other stakeholders to enhance service delivery and resource availability
- Advocate for children's health and wellness initiatives within the community and at the policy level
- Represent the organization at community meetings, conferences, and other professional events
- Develop strategic alliances to expand access to pediatric specialty services and community resources
- Quality Assurance and Compliance
- Ensure compliance with all relevant regulations, policies, and standards pertaining to children's health services, including New York State Health Home requirements
- Implement quality improvement initiatives to enhance service delivery and client satisfaction
- Conduct regular assessments and audits to ensure adherence to program protocols and standards
Maintain accurate documentation and reporting systems for regulatory and accreditation purposes
Qualifications
- Master's degree in social work, Public Health, Psychology, or related field
- Current New York State licensure (LCSW, LMHC, or equivalent) preferred
- Minimum of 5 years of experience in children's services, healthcare, or social services
- Minimum of 3 years in a supervisory or leadership role
- Strong knowledge of the Health Home model, child development, and best practices in pediatric healthcare
Experience working with diverse, underserved populations
- Proficient in data analysis and program evaluation methodologies
