Responsibilities
The Nurse Navigator will be responsible for coordinating service and identified patients through a complete health system by assisting with access issues, identifying resources, providing educational material, and developing relationships with service providers and community. The Nurse Navigator coordinates the care in a timely and efficient manner, identifying and eliminating barriers to care. The Nurse Navigator is a consistent care coordinator throughout the continuum of care and assess the physical, psychological, and social needs of the patient enhancing patient outcomes, increasing satisfaction, and reducing cost of care. The Nurse Navigator is responsible for community education and screening as well as monitoring community outreach and promoting survivorship.
Qualifications
EDUCATION, EXPERIENCE, TRAINING
1. Current and valid state RN License.
2. Current BLS certificate upon hire and maintain current.
3. Bachelor of Science in Nursing (BSN) preferred.
4. Minimum of two (2) years of nursing experience.

Pay Transparency
Holy Family Medical Center offers competitive compensation and a reasonable compensation estimate for this role is $43.09 an hour. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
