Position Summary:
Provide care monitoring services to assigned patients and functions as a member of the interdisciplinary team. Assists with coordination of care for patients who are receiving services at Peninsula Outpatient Centers. Responsibilities
- Completes nursing assessments in a timely and quality oriented manner using a holistic approach.
- Participates in screening of individuals seeking referral for mental health services and triages appropriately.
- Collaborates with multidisciplinary team members in the evaluation of treatment effectiveness for patients served.
- Completes CRG and TPG assessments as required.
- Assesses needs of patients: living arrangements, financial, vocational/ educational, health, social support, leisure/recreational and emotional/mental health, including a mental status exam-as necessary. Assists patients in establishing and building support system and resources.
- Provides on-going linkage, monitoring, planning and evaluation and advocacy for patients.
- Proactively attends meetings and trainings as indicated in the Peninsula Staff Development Plan, and as required to maintain license.
- Provides liaison and networking with community agencies as appropriate.
- Provides patient assessment in preparation for MD visits.
- Functions as the physician’s scribe as needed.
- Conducts individual and group education for patients regarding medication and treatment compliance and documents results.
- Assesses medication effectiveness and side effects and provides patient and/or family education on illness, appropriate medication use and side effects. Communicates with physicians as appropriate regarding patient response.
- Employs therapeutic communication principles when communicating with patients.
- Administers medications with accuracy and safety.
- Employs standard precautions and aseptic techniques when appropriate.
- Provides emergency first aid to consumers, visitors and staff.
- Acts as patient advocate, obtaining prior approval, interacting with pharmacies to assure patients receive medications appropriately.
- Develops policies and procedures as needed.
- Participates in continuous quality improvement activities and may lead quality improvement task forces.
- Maintains a positive attitude, serves as a role model to others and own team and strives to exemplify Peninsula’s values.
- Projects a customer oriented mentality and responds to customer inquiries in the established clinic time frames.
- Continuously works to see that licensure standards are met and that quality improvement is occurring.
- Performs injections or venipuncture as indicated by MD orders.
- If qualified, functions as a RNC Care Manager and adheres to the program guidelines and policies of the RNC Care Manager Program.
- Performs other related duties as assigned.
Qualifications
Minimum Education:
Diploma or Associate Degree in Nursing. Bachelor of Science in Nursing preferred.
Minimum Experience:
One (1) to two (2) years experience preferred.
Licensure Requirements:
Required. Holds a current license as a Registered Nurse issued by the licensing board for the State of Tennessee. Certification by the American Nurses Association as a Psychiatric and Mental Health Nurse preferred.
