About Legacy Hospice Services
Legacy Hospice Services delivers high-quality, compassionate, home-based care to adults, children, and their families throughout the Portland metropolitan area and the Willamette Valley.
We also provide hospice care in residential and assisted living communities, adult foster homes, and skilled nursing facilities - ensuring support and dignity for patients wherever they call home.
Responsibilities
- Performs professional community health nursing visits and services for terminally ill patients and their families.
- Provides pain and symptom management tailored to patient needs.
- Serves as a member of the Hospice Interdisciplinary Team (IDT); coordinates and assists in implementing the plan of care for the patient and their family/caregiver.
- Completes nursing assessments and delivers palliative treatment and symptom management.
- May conduct initial visits to admit patients and initiate or revise the hospice plan of care.
- Prepares timely and accurate clinical documentation.
- Participates in IDT meetings and case conferences as appropriate.
- Collaborates with the care team to coordinate services and ensure continuity of care.
Qualifications
Education:
- As required by licensure.
- BSN or degree in progress strongly preferred.
- BSN may be required for specific assignments; experienced RNs without a BSN may be hired based on clinical experience with approval of the site CNO.
Experience:
- One-year registered nursing experience or completion of either the Legacy Health RN Residency program or a nursing residency program that is equivalent in content, curriculum and duration.
- Hospice experience or one year medical/surgical acute care or long-term skilled nursing experience preferred.
Skills:
- Working knowledge of and the ability to apply the basic principles and techniques of community hospice nursing in a variety of home settings.
- Must possess excellent clinical nursing skills and be able to utilize problem-solving techniques.
- Ability to establish and maintain effective working relationships with professional groups, volunteers and the general public.
- Must possess excellent communication skills both verbal and in writing.
- Keyboard skills and ability to navigate electronic systems applicable to job functions.
Licensure
- Current RN license required.
- BLS for Healthcare Providers certification from the American Heart Association required.
- Valid driver’s license required.
- Certified Hospice and Palliative Nurse (CHPN) from the National Board for Certification of Hospice & Palliative Nurses (NBCHPN) preferred.
Pay Range
