Job Summary and Responsibilities
As a Registered Nurse with Commonspirit Health at Home, you’ll be at the heart of patient care delivering expert clinical services, coordinating with multidisciplinary teams, and building meaningful relationships with those you serve. Responsibilities:
- Deliver compassionate, high-quality nursing care tailored to each patient’s needs.
- Develop and adjust individualized care plans to ensure optimal patient outcomes.
- Collaborate with physicians, therapists, and other healthcare professionals.
- Supervise your own caseload, nurturing the growth and development of LPNs, LVNs, and Home Health Aides, to facilitate and cultivate a team of caregivers.
- Provide education and support to patients and their families.
- Maintain thorough documentation to meet regulatory and operational standards.
- Utilize the latest technology and best practices to enhance patient care.

- Will support both Home Health and Hospice patients
- Flexible schedule. Either 4-5 shorter days or 2-3 longer days that must include Saturday and Sunday-24 hours/ week
- Graduate of an accredited nursing program; must hold an active RN license in the state served.
- At least 1 year of nursing experience (Home Health preferred).
- Strong critical thinking, clinical expertise, case management, and a passion for patient care.
- CPR/BLS certification
- Valid driver’s license
