Overview

The Hospital Pharmacist is responsible for preparing, dispensing, and monitoring medications to ensure safe, effective, and cost-efficient drug therapy for our pediatric patient population. Our pharmacists work closely with physicians, nurses, and other healthcare professionals to optimize patient care, provide drug information, and ensure compliance with all applicable laws, regulations, and hospital policies.

The Pharmacist reports to the Director of Pharmacy.

Schedule: This position will have variable shifts consisting of 7.5 hours per day throughout the week.

  • Schedules vary from 7am-3pm, 8am-4pm, 9am-5pm, 11am-7pm, 1pm-9pm.
  • Schedule includes rotational weekend coverage as well as rotational holiday coverage.
  • Schedule will also include clinical coverage and staff coverage.
  • On-call at nights rotated with all pharmacists when pharmacy is closed weekdays/weekends/holidays.

Responsibilities

  • Reviews medication orders and profiles for allergies, interactions, contraindications, drug incompatibilities, therapeutic duplications and appropriateness of drug therapy.
  • Regularly consults with prescribers concerning drug availability, formulary status, proper drug use, side effects and incompatibilities. Obtain clarifications or suggestions for changes or additions to medication orders.
  • Reviews laboratory data and suggests changes to medication therapy when appropriate.
  • Formulates clinical interventions and consults with practitioners to create an appropriate medication regimen. Documents all interventions.
  • Reviews Admission and Discharge Medication Reconciliation to ensure accuracy at transitions of care.
  • Detects, monitors, documents, and reports adverse drug reactions and medication errors.
  • Promptly fills prescriber orders after reviewing the appropriateness of the order for the patient.
  • Accurately checks patient specific medication cassettes prior to delivery.
  • Accurately checks all Omnicell Normal, Supplemental and Critical Low restocks prior to delivery.
  • Demonstrates knowledge of the JCAHO Medication Management Standards and Patient Safety Goals
  • Competently compounds special orders or dosage forms per acceptable techniques as dictated by current industry standards.
  • Ensures Prepackaging and Repackaging Policies and Procedures are adhered to.
  • Ensures proper storage conditions for all pharmaceuticals
  • Reports drug-food interactions to the dietician.
  • Maintains an accurate perpetual Controlled Substance inventory and ensures adequate control and documentation of Controlled Substance Distribution in accordance with State and Federal laws.
  • Provides accurate and clinically relevant drug information to prescribers, nurses and other health professionals.
  • Orders and verifies TPN utilizing the CAPS computer program
  • Prepares IV admixtures and Chemotherapy products. Adheres to USP 797 Policies and Procedures.
  • Checks refrigerator and freezer temperature reports daily.
  • Assists in assessing levels of pharmacy stock, ordering supplies, and removing overstocks and outdates.
  • Follows Hours On-Call procedures
  • Checks After Hours log and assures Omnicell Night Cabinet is replenished.
  • Check all Omnicell Override reports.
  • Participates in Pharmacy Quality Assurance and Quality Control activities
  • Works on special projects as they may be assigned
  • Supervises Pharmacy Technicians
  • Makes recommendations for cost containment and improvements in medication safety.

Qualifications

  • Minimum 1 year experience or internship in hospital pharmacy.
  • Knowledge in areas of Unit Dose, IV Admixtures, TPN, Chemotherapy, CPOE and Automated Dispensing Machines.

Education, Licensure, and Certification

  • Graduate of an accredited School of Pharmacy with a BS or PharmD pharmacy degree.
  • Licensed or license eligible by the New York State Department of Education as a Pharmacist.

Physical Requirements

  • Standing, walking, stooping. 
  • Ability to lift supplies/equipment up to 50lbs.
  • Ability to meet flexible scheduling demands and patient care needs.
