Job Summary

Position: Community Healthcare Advocate

Job Summary: The Community Healthcare Advocate works directly with individuals and communities to facilitate access to healthcare services, health education, and resources. They serve as a liaison between healthcare providers, community organizations, and individuals, particularly those facing barriers to healthcare access. The advocate empowers individuals to make informed decisions about their health, ensuring they receive necessary care, prevention services, and follow-up support.

Responsibilities

  • Educate members about importance of regular health screening, health checkup, and preventive care.
  • Assist members in navigating the health system, including but not limited to Co-ordination of care.
  • Connect members to community resources, such as housing assistance, healthcare, food insecurity programs.
  • Provide education to members and care givers to address systemic barriers that may prevent members from accessing needed support.
  • Identify and engage hard- to-reach populations to engage them in health services.
  • Provide immediate assistance to navigate during emergency health situations.
  • Timely monitor and follow-up on member’s progress and support plans as necessary by conducting virtual / in person timely outreach.
  • Maintain detailed and confidential records as per HIPPA compliance.
  • Work with team to provide coordinated and comprehensive care.
  • Work with the Administrative staff to provide support in the day to day operational activitie
  • Engage with local communities to raise awareness about available healthcare services.
  • Conduct informational sessions on health promotion, disease prevention, and healthy living.
  • Provide culturally sensitive health education to diverse populations.
  • Advocate for patients with healthcare providers to ensure appropriate care.

Qualifications

  • A degree in public health, social work, nursing, or a related field is required.
  • At least 2-3 years of experience in healthcare, community outreach, or social services.
  • Experience working with diverse populations and knowledge of health disparities and community-based resources.
  • Cultural competence and the ability to work with people from diverse backgrounds.
  • Knowledge of healthcare systems, insurance, and public health programs.
  • Excellent communication and interpersonal skills to include the ability to develop relationship with members and caregivers.
