The Licensed Practical Nurse (LPN) provides direct nursing care to patients in accordance with professional standards and established policies and procedures. The LPN collaborates with the interdisciplinary care team to support patient treatment plans, promote positive patient outcomes, and ensure a safe, therapeutic environment. This position requires strong clinical skills, effective communication, and a commitment to delivering high-quality patient care.
Responsibilities
- Provide direct patient care under RN or provider supervision.
- Support development and implementation of patient care plans.
- Administer medications and treatments per protocol.
- Monitor and document patient condition; report changes promptly.
- Assist with assessments, ADLs, mobility, and safety needs.
- Maintain accurate and timely EHR documentation.
- Educate patients and families on care plans and instructions.
- Collaborate with interdisciplinary team members to ensure coordinated care.
- Adhere to all regulatory, safety, and infection control standards.
Qualifications
EDUCATION, EXPERIENCE, TRAINING
1. Current and valid state license as a Licensed Vocational Nurse.
2. Current BLS certificate upon hire and maintain current.
3. IV certification upon hire; Blood Withdrawal (BW) certification preferred.

Holy Family Medical Center offers competitive compensation and a comprehensive benefits package that provides employees the flexibility to tailor benefits according to their individual needs. A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $31.00 to $40.00 per hour. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
