We are seeking a compassionate and professional Hospice Outreach Counselor to join our team in San Diego, CA. This role is ideal for an LVN (preferred), Chaplain, or Social Worker with hospice experience, who is passionate about supporting patients and families during a critical time in their lives.
In this role, you will serve as a key liaison between our organization, patients, families, and community partners—educating on services, securing consents, and ensuring emotional and informational support throughout the care journey.
Responsibilities
- Establishes and maintains positive working relationships with current and potential referral and payer sources
- Builds and monitors community, customer, and patient perceptions
- Introduces and completes Explanation of Benefits with patients and/or patient families
- Ensures consents are completed in accordance with appropriate guidelines and regulatory government agencies
- Maintains comprehensive working knowledge of service markets including government agencies, major payer groups, key referral sources and competitor market positioning
- Maintains comprehensive working knowledge in the field of marketing and sharing information with appropriate organization personnel
- Participates in the development of the individualized plan of care
- Counsels patient and family/caregivers and/or connecting to appropriate team resources as needed for stress and other coping mechanisms
- Occasional on-call responsibility as appropriate for discipline
- Ensures compliance with all state and federal legal and regulatory requirements
- Additional responsibilities as assigned
Qualifications
- Master’s degree in Social Work, Bachelor’s degree in Social Work, Licensed Vocational Nurse or Chaplain
- One year experience in health care marketing, preferably in hospice care
- Ability to deal tactfully with customers and the community
- Knowledge of corporate business management, governmental regulations, and Joint Commission standards
- Demonstration of good communication and public relations skills
- Demonstration of autonomy, organization, assertiveness, flexibility, and cooperation in performing job responsibilities
