Frequently asked questions

Providers in this role frequently coordinate with multidisciplinary teams within nursing homes and assisted living settings, balancing independent decision-making with collaborative patient care to ensure seamless transitions and tailored post-acute treatment plans.

Board certifications recognized nationally, such as those from the American Nurses Credentialing Center or the National Certification Corporation, are crucial. Additionally, active Florida licensure and DEA certification support compliance and broaden practice scope in post-acute care.

Daily duties include conducting new patient admissions, follow-ups, acute visits, discharges, and regulatory assessments while managing lab orders, medication reviews, patient education, and coordinating referrals to optimize recovery outcomes.

Unlike acute care providers focusing on immediate hospital treatment, post-acute care providers emphasize ongoing recovery management in skilled nursing or assisted living environments, requiring adaptability to long-term care protocols and patient education.

At First Physicians Group, advanced practice providers benefit from a supportive nonprofit culture offering comprehensive benefits, sovereign immunity, and opportunities to engage in public service loan forgiveness programs, fostering professional growth within post-acute care.

This position uniquely combines independent fieldwork with collaborative multidisciplinary care, emphasizing evidence-based practices in skilled nursing facilities, supported by competitive compensation and a nonprofit employer focused on community health.

Sarasota's aging population elevates demand for skilled post-acute providers, making this a sought-after specialty. Healthcare facilities increasingly seek qualified APRNs and PAs to manage complex transitions from hospital to long-term care settings.

Given Sarasota's spread-out care facilities, providers should anticipate travel between multiple nursing homes and assisted living centers during work hours, requiring efficient route planning to maintain timely patient visits within typical office hours.

Advanced practice providers specializing in post-acute care in Sarasota typically earn between $105,000 and $130,000 annually, reflecting regional demand, required certifications, and specialized skills in managing skilled nursing facility patients.

Providers manage patient admissions, follow-ups, acute and discharge visits, order and interpret diagnostic tests, adjust medications, educate families, and collaborate with facility staff to ensure comprehensive post-acute care tailored to individual patient needs.
