Overview
Responsible for making home visits to program participants. Their primary function will be to work with children and families on enhancing the parent-child interaction. They will utilize activities that will focus on various issues such as child abuse prevention, child development and child friendly activities to encourage learning.
Responsibilities
- Establish and maintain positive relationships with patients, visitors, and other employees. Interacts professionally, courteously, and appropriately with patients, visitors and other employees. Behaves in a manner consistent with maintaining and furthering a positive public perception of Bronx-Lebanon Hospital Center and its employees.
- Contributes to and participates in the Performance/Quality Improvement activities of the assigned department. Contribution and participation includes data collection, analysis, implementation of and compliance with risk management and claims activities, support of and participation in Continuous Quality Improvement (CQI) teams, consistent adherence to the specific rules and regulations of the Bronx-Lebanon Hospital Center (a) Safety and Security Policies, (b) Risk Management: Incident and Occurrence Reporting, (c) Infection Control Policies and Procedures and (d) Patient and Customer Service.
- Responsible for initiating and maintaining regular contact with families primarily in their homes.
- Provides intensive services and ensures completion of goals. These intensive services will focus on promoting positive parenting skills that will center on positive parent-child interaction and enhance the knowledge of child development of program participants utilizing various program activities.
- Conducts assessment with the input of the family to determine service needs and develop goals utilizing the Family Support Plan.
- Escorts families to agencies and medical visits when necessary.
- Responsible for documenting all contact with families as they pertain to all service delivery in case records and in other required data collection instrument.
- Assists with special projects and performs other duties as delegated by the Program Manager and Supervisor.
Qualifications
- Experience working within childcare facility such as a Day Care Center/Head Start Program or working with high-risk families preferred.
- Completion of certificate program in Child Care
- High School or GED
- Computer literate
