Job Summary and Responsibilities

Incumbent participates as a member of the health care team, working with professional and non-professional staff and associated professionals in providing care which provides optimum home care and continuity of care to referred patients, addressing the age of the patients served.


Job Requirements

Minimum Requirements:

  • One year experience as a physical therapist required
  • BLS and current CA Physical Therapy License required.
  • CAR INS;DRV LIC;

Knowledge and ability to perform current physical therapy techniques, knowledge of physical therapy standards, procedures and principles, ability to operate physical therapy equipment for patient care, ability to implement practical application of knowledge, ability to work independently and autonomously and ability to teach and instruct patients, caregivers and staff required Computer skills necessary for position. 

Please be advised that this location requires quarterly American Heart Association (AHA) Resuscitation Quality Improvement (RQI) competency certification post-hire. This ongoing competency is required through resources internal to the facility; outside RQI competency resources are not acceptable for CommonSpirit Health compliance requirements.
