Responsibilities
- Perform patient evaluations and develop personalized treatment plans.
- Regularly reassess treatment effectiveness and participate in staffing meetings and family conferences.
- Conduct discharge evaluations, recommend home and community safety modifications, and provide education and referrals.
- Communicate patient progress and needs to the treatment team, physicians, patients, and families.
- Collaborate with other disciplines to coordinate care and enhance patient outcomes.
Qualifications
- Degree in Physical Therapy from an accredited program.
- Active Physical Therapist license in the practicing state.
- CPR or Basic Life Support certification per state regulations.
- At least one year of physical therapy experience preferred.
- Knowledge of rehabilitation techniques for complex neurological injuries.
- Strong verbal and written communication skills.
- Ability to lift 50 pounds; safely move/transfer patients up to 100 pounds.
- Physical stamina: fine motor skills, visual acuity, extended standing/walking, pushing, pulling, bending, stooping, reaching, kneeling, and positioning.
- Health screenings or exams may be required per location or state policies.
