Job Summary and Responsibilities

In this role, you will provide vital support to the Director of Clinical Services and our clinical staff to optimize patient care. You will act as a clinical intermediary and ensure seamless communication between healthcare providers, patients, and administrative departments.
Key Responsibilities:
- Assist in the completion of daily activities to enhance the efficiency of clinical staff.
- Update physician orders and communicate orders or changes to the appropriate field staff.
- Entry of ICD-9 codes and manage OASIS assessment data entry accurately.
- Liaise with the corporate Revenue Cycle Department to address queries and resolve issues as required.
- Audit clinical charts and maintain clinical documentation, such as discharge summaries, in accordance with company policies.
- Communicate effectively with external service providers (e.g., HME, supply vendors).
- Assist in training and onboarding new staff members, identifying clinical training needs.
- Participate in case conferences for interdisciplinary care coordination.
- Perform client visits as directed and offer clinical expertise to internal teams.

- Current LPN license in good standing for the state serviced.
- Minimum of two years' experience in a clinical or support role.
- Strong clinical skills and familiarity with multiple software programs, particularly Windows-based applications.
- Excellent organizational, interpersonal, and communication skills.
- Proven ability to work collaboratively within a multidisciplinary team.
- Strong attention to detail with effective time management capabilities.
- Valid Driver’s license and auto liability insurance as per organizational policy.
