This facility offers Orthopedic Rehab, Speech Therapy and Physical Therapy. While also provide specialty programs including Palliative Care, Respite Care and Wound Care in addition to Long Term Care and Skilled Nursing. Pay Ranges depend on Job Title, years of experience and eligibility for our compensation programs such as Pay-In-Lieu of Benefits (adds 11% to your base pay) and Shift Differentials.
PT: $34.98 - $52.47 / Hour
PTA: $23.39 - $35.08 / Hour
LNA : $19.80 - $27.86 / Hour
LPN: $27.60 - $41.28 / hour
RN: $40.82 - $55.22 / hour
Responsibilities
RN - Registered Nurse:
- RN Team Leaders assist in ensuring the general health and well-being of our residents by providing direct nursing care. Direct nursing care of residents encompasses planning, developing, organizing, implementing assessing, and evaluating the care of residents to ensure that the highest degree of quality care and regulatory compliance is maintained at all times.
LPN - Licensed Practical Nurse:
- Licensed Practical Nurses (LPNs) are indispensable members of our interdisciplinary team and are charged with managing the daily clinical regimen of each resident and sub-acute patient. Our LPNs enjoy a unique opportunity to gain extensive clinical experience and provide advanced care in a home-like setting.
LNA- Licensed Nursing Assistant
- Licensed Nursing Assistants (LNA) provide personal and nursing care to meet the physical and emotional needs of residents in Elderwood skilled nursing facilities (SNF). They are under the direction of a licensed nurse. State specific certification is required.
PT - Physical Therapist:
- Physical Therapy team members assist with ensuring the health and well-being of our residents by providing comprehensive and quality therapy services. This position is responsible for administering skilled physical therapy treatments ensuring that the highest degree of quality resident care is maintained at all times.
PTA- Physical Therapy Assistant:
- PTA team members assist with ensuring the health and well-being of our residents being responsible for administering skilled physical therapy treatments as developed and recommended by a Registered Physical Therapist, ensuring that the highest degree of quality care is maintained at all times.

- Credentials verifying appropriate certificate and license in New York State- RN, LPN, LNA, PT, PTA; current State license/Certificate required.
- Ability to communicate well verbally and in writing required.
- Qualities of maturity, diplomacy, and ability to work well with others required. Caring, respectful attitude toward residents required.
- Physical stamina for constant activity required.
- Ability to perform tasks to established standards of excellence required.

- These position requires regular interaction with residents, coworkers, visitors, and/or supervisors.
