Position Summary:

Administers routine services in caring for the subacute, convalescent and chronic residents of the Nursing Home. Participates in planning, implementation and evaluation of nursing care. Directs and supervises the CNAs assigned to his/her wing.

Responsibilities

  • Maintains continuity of resident care during respective tours of duty and prioritizes nursing tasks.
  • Assigns residents to CNA. Supervises and assists nursing staff in planning care for each resident. Assists in counseling CNAs as necessary.
  • Assists with orientation of new personnel.
  • Make frequent rounds on residents. Observes and records symptoms and/or reactions to treatments and changes in condition. Informs physicians of changes in resident’s condition and follows up orders as necessary.
  • Maintains good relations with visitors, families, and other people while on duty. Works closely with supervisor in dealing with personnel, residents, and resident’s family situations. Relays pertinent information concerning resident’s care/condition to family member/responsible party.
  • Delegate authority to other licensed persons as necessary. At the end of shift, reports to relief personnel all pertinent information for continuity of care and assures follow through care as needed
  • Starts IV, administers IV fluids and IV meds.
  • Inserts, irrigates, and feeds residents through nasogastric and gastrostomy tubes.
  • Administers aerosol treatments as ordered.
  • Performs all facets of admission and discharge of residents. Begins care plan within 24 hours of admission and completes admission assessment within 8 hours of admission.
  • Administers medications to residents (orally, IM, and SubQ, topically and intradermally). Orders medications for use on unit as needed.
  • Maintains level of stock drugs, equipment and supply needs. Assures that all meds, IV fluids, etc. are not expired. Orders stock from purchasing on a weekly and as needed basis.
  • Adheres to safety and infection control standards, and policies and procedures. Demonstrates use of Standard Precautions in all aspects of nursing care.
  • Supervises private sitters assigned to residents on units.
  • Maintains and performs sterile and clean dressings, and monitors wounds including depth, circumference, edge approximating, and tissue granulation formation. Charts appropriately and does dressing changes as order by physician.
  • Does a comparison of present resident care with documented plan of care and compares attainable goals. Completes a monthly summary on assigned residents using this information.
  • Orders treatment (such as laboratory tests, x-rays, or diets, etc.) online.
  • In the absence of Nursing Supervisor and/or DON, assures adequate staffing by covering for any call-offs.
  • Assures that 2 licensed persons sign off all physician’s orders (on the physician’s order sheet and on the MAR) and that a 990 form is completed for all verbal or telephone orders. Transcribes physician orders.
  • Assists in maintain a safe environment for all residents, which may include reporting to appropriate managerial staff equipment that is out of order, suspicion of resident abuse, mistreatment, neglect and reporting of suspicion of criminal activities.
  • Demonstrates age-specific competencies.
  • Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.
  • Performs other duties as assigned.

Qualifications

Minimum Education:           

None specified; however, must be sufficient to meet the standards for achievement of the below indicated license and/or certification as required by the issuing authority.

 

Minimum Experience:         

No prior experience as a LPN/LVN needed.

Licensure Requirement:      

Currently licensed by the Tennessee Board of Nursing as a Licensed Practical Nurse (LPN).
