SSM Health Home Health Care is seeking a Full-Time Physical Therapist Assistant to join our Home Care Team servicing our Central territories of St. Louis! We are looking for a PTA who loves the experience of treating and progressing patients in their home environment. Responsibilities
• Assists the therapist in data collection including appropriate tests and measurements for initial evaluation and communicates status updates, as directed by the therapist, within their scope of practice. • Assists in the implementation of the therapy plan of care for their assigned patients including performance of therapy procedures and related tasks that have been selected and delegated by the supervising therapist.• Progresses treatment techniques and procedures as patient’s status and functional potential changes in conjunction with the specific therapy plan of care designed by the supervising therapist.• Identifies, responds and reports to supervising therapist any changes in the patient’s physical/mental status and/or tolerance to treatment.• Under direction of supervising therapist, coordinates treatment program in conjunction with other clinical/therapeutic services, as well as available community resources. Attends appropriate clinical meetings to facilitate coordination.• As delegated by supervising therapist, provides for patient education to include development of comprehensive and progressive home programs with instruction to patient, family and other caregivers as appropriate to facilitate a safe and effective discharge• As delegated by the supervising therapist, prepares for patient discharge inclusive of ordering of/or fabrication and adaptation of necessary equipment, as well as recommendations for therapeutic services ensuring the continuum of care.• Assists with direction of functional training classes and/or programs under the supervision of licensed therapist as assigned.• Assigns routine tasks to aides and clerical staff related to their job function in conjunction with supervising therapist. • Demonstrates flexibility in all aspects of caseload management including accepting coverage. Effectively and efficiently manages individual, concurrent, and group treatment sessions based on patient care needs. • Completes medical record documentation in a clear, concise, accurate and timely manner, inclusive of daily documentation and charge entry using approved formats in compliance with state, federal and intermediary guidelines and in accordance with the facilities policies and procedures. Ensures care is provided under signed, valid physician orders and within insurance approval, as indicated and necessary for clinical care of the patient. Ensures that daily documentation is cosigned by supervising therapist. • Under the direction of supervising therapist, participates in therapy assistant student program as assigned and appropriate, providing a positive learning environment for therapy assistant program students with appropriate supervision. Will attend all student lectures, and begin developing public speaking skills required to facilitate student lectures.• Actively creates an individualized professional development plan with clinical management. Identifies and initiates own professional growth, actively pursuing professional development through continuing education, self study and involvement in professional activities and organizations. Applies learned information in order to meet the strategic objectives of the organization.• Actively participates in patient and professional advocacy efforts that may include lobbying activities, health fairs, community level events, professional memberships, fundraising/charitable activities.• Adheres to discipline specific State Practice Act and other regulatory agencies. Maintains professional licensure in good standing and completes all state mandated continuing education requirements. Performs in an ethical manner in accordance with discipline specific professional code of conduct. • Understands the organization’s performance improvement methodology to improve quality of services provided by this position. Demonstrates awareness of organizational performance improvement initiatives and follows through with the action plans. Offers feedback through staff meetings, focus groups or direct supervisor in order to maximize performance improvement outcomes. • Performs other duties as requested.
Qualifications
Home Care Requirements:
- Current state licensure or eligibility for state licensure as an Physical Therapy Assistant required.
- Certified BLS or completion in first 90 days of employment required.
- One or more years of clinical experience
- Valid state Driver’s License.
