Education/Experience/Licensure
- Graduate of a medical school accredited by LCME or COCA and completion of a ACGME accredited residency in a primary care field (family medicine, internal medicine, or pediatrics)
- 5+ years of primary care practice experience. Experience in integrated settings, with MAT, and/or delivering primary care to Severely Mentally Ill (SMI) population preferred. Demonstrated experience in caring for underserved populations strongly preferred.
- 1+ years of administrative or supervisory experience in a medical department (preferably in an ambulatory care setting) strongly preferred.
- Must have an active, unrestricted licensed to practice medicine in the State of Indiana
- Must have a current Drug Enforcement Administration (DEA) certificate and State of Indiana Controlled Substance Registration (CSR)
- Must meet credentialing requirements of Affiliated Hospitals
- Must be Board Certified
- Must have a current BLS certification, or renew upon hire
