Responsibilities
What you will do: Responsibilities listed include but not limited to:
- Delivering attentive and personalized care following each client's unique care plan
- Monitoring and analyzing clients' vital signs, promptly reporting any notable findings to the healthcare team
- Administering medications, feedings, oxygen, and specialized care according to individual care plans, ensuring each client's specific needs are met.
- Collaborating with other healthcare professionals such as Physical Therapists (PT), Occupational Therapists (OT), and Speech-Language Pathologists (SLPs) to ensure comprehensive care routines are established and maintained.
- Maintaining accurate documentation of all nursing actions, effectively communicating and documenting progress and outcomes for established goals.
Qualifications
What you will need:
- Proof of current license as an LPN or LVN in good standing from the state(s) of practice
- Current CPR certification
- Ability to work in a home-like setting.
- Be able to bend stoop, push, pull, sit, and walk for periods of time
- Able to utilize proper body mechanics while lifting 50 pounds
- Knowledge of individual service plans, including understanding of client rights and health needs
- Must have completed post-secondary coursework to facilitate licensure as an LPN or LVN
