Job Summary
We are seeking an operations manager to join our ACO team to oversee day oversee the day-to-day operational functions of our Provider Accountable Care Organization (ACO). This individual will play a critical role in coordinating cross-functional activities that drive performance improvement, ensure compliance with ACO quality and cost metrics, and support provider engagement. The ideal candidate has a strong background in healthcare operations, project management, and value-based care delivery models.
Responsibilities
1. Project management:
a. Lead the planning, implementation and tracking of value-based care initiatives in daily operations
b. Develop and maintain project timelines, deliverables and achieve KPIs aligned with organizational goals.
c. Coordinate with cross-functional teams to achieve project objectives.
2. Planning and strategy execution:
a. Develop and implement tactics to achieve strategic goals (e.g., reduce ED utilization and improve patient engagement)
b. Develop and implement strategies for engagement and continuity of care for the aligned population to model.
c. Develop risk stratification models and prioritize care interventions based on need and impact.
d. Collaborate with leadership to align clinical practices with quality metrics and reimbursement models.
3. Clinical operations support:
a. Develop and implement workflows to support team based and patient centered care.
b. Integrate roles and responsibilities of each team member in successfully driving the tasks in order to achieve goals.
c. Identify care gaps and support clinical team in addressing operational issues.
d. Be a bridge between Clinical staff, leadership and patients by regular meetings to achieve high quality and smooth daily operation.
e. Coordinate with community partnerships to address social determinants of health and behavioral health in the aligned population
4. Data and Reporting:
a. Develop innovative approach to create reporting along with working with data analytics team to track and report key performance indicators on daily operation.
b. Evaluate and implement data insights to drive continuous quality operation.
c. Skilled in using MS-Office (PowerPoint, excel, etc.) for reporting and analysis for daily operation
d. Maintain records of goals, timelines, outcomes and learnings.
5. Compliance & Reporting:
a. Ensure compliance with CMS regulations, including timely submissions of required documentation and data.
b. Support audit readiness and maintain operational documentation and protocols.
6. Technology Integration:
a. Work with IT teams to enhance EHR use and develop user friendly functions for clinical team.
b. Operationalize integration of population health platforms, predictive analytics and remote monitoring tools
Qualifications
Additional Responsibilities:
- Bachelor’s degree in Healthcare Administration, Business, Public Health, or related field required; Master’s degree preferred (e.g., MHA, MBA, MPH).
- Previous project management experience / Medical background preferred
- The ability to understand medical information, facility policies and procedures and present this information to appropriate personnel
- Excellent organizational, communication, and interpersonal skills.
- Ability to work collaboratively with diverse provider groups and administrative teams.
- Strong Microsoft excel skills
- The ability to be a team player
Salary: $65,000-$80,000
