This role provides individuals and families with dependable and age-appropriate Family Support. This is a direct service position, responsible for the provision of individualized support to participants. This role provides direct support or services with a higher level of skill or experience, contributing to our service recipients' care and support while maintaining quality, service excellence and ethical standards. This role may train or supervise direct support employees and works closely with a multidisciplinary team to implement individualized care plans that align with each person's goals and needs.

Responsibilities
- To participate in a pre-placement interview with Family Services and the family/individual to be served to develop a specific individual/family an individualized plan of support.
- To provide personal care to the individual if needed.
- To complete detailed time sheets, including a description of activities, obtain family signatures on time sheets and submit these time sheets in accordance with the Seven Hills payroll schedule.
- To collect and submit all receipts as required.
- To provide one to one support to individuals without exceeding allotted hours.
- To complete orientation and all trainings as required by Seven Hills Family Services, Inc., and the Department of Developmental Services. Maintain all required documentation and certifications.
- To attend specific job-related training as assigned.
- To maintain a working knowledge of the Seven Hills Family Services Support Worker Manual and Seven Hills Policies and Procedures.
- To be knowledgeable of community activities.
- To support each individual’s existing relationships and increase their networks of friends and acquaintances.
- To communicate regularly with families, individuals and Seven Hills Family Support staff.
- Consistently maintain an agreed upon schedule.To report any change and/or concerns impacting the individual/family to the Seven Hills contact in a timely manner.
- To engage individuals in community activities based on the person’s interests as expressed in their Individual Support Plan (ISP) and provide opportunities to explore potential new interests in the community.
- To provide a high level of care to the individuals, including but not limited to safety, well being and dignity.
Qualifications
Education & Experience:
Must be at least of eighteen (18) years of age.
- Associate's degree preferred
3 – 5 years of related experience
Skills and Knowledge:
- Standard driver’s license – class C or class D
An interest in working with individuals with a disability.
- Knowledgeable of creative activities appropriate for the age and capabilities of the individual being supported.
