As a Hospice LPN You will:
- Give hands-on care to patients nearing the end of life, following state nursing rules.
- Follow the care plan after assessing the patient’s needs.
- Keep clear and timely records of the care provided.
- Teach and support patients and their families about the illness and care plan.
- Help coordinate care by communicating with the hospice team (like aides, chaplains, social workers).
- Manage pain and symptoms, checking how well treatments are working.
- Take part in training and quality improvement activities to grow professionally and improve care.

Qualifications
- Graduate of an accredited practical nurse or vocational nursing program.
- Three years nursing experience. Community health/hospice or medical/surgical experience is preferred.
- Currently licensed as an LPN/LVN in the State.
- Comply with accepted professional standards and practice.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
- Possess and maintain current CPR certification.
- Understand philosophy of hospice concept. Also understand needs of the terminally ill.
- Use own knowledge and experience base and other resources as necessary to make logical decisions and solve problems.
- Demonstrate good verbal and written communication, and organization skills.
