Job Summary and Responsibilities
We are seeking a skilled and compassionate Occupational Therapist to join CHI Franciscan Home Health team. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.
Responsibilities:
- Assess patients’ physical, cognitive, and psychosocial abilities to determine therapy needs. Develop and implement personalized treatment plans that may include exercises’, adaptive techniques, and use of assistive devices to improve patients functional abilities.
- Teach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
- Maintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
- Monitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.
- Collaborate and communicate patient’s change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

- Bachelor's degree from an accredited school approved by the American Occupational Therapy Association
- Active registered OT license in the state which you practice
- Must obtain and maintain current CPR/BLS Certification
- One-year Occupational Therapy experience in a clinical setting.
- Current Drivers License
- Comply with federal, state and local laws and regulations
