Responsibilities
Provide respectful, person-centered support to individuals in their homes and communities, promoting dignity, independence, and self-direction
Implement and monitor Individual Support Plans (ISPs), behavior support plans, and therapeutic routines (e.g., physical therapy, personal care) as outlined
Assist individuals in developing life skills, managing household responsibilities, and participating in meaningful community activities
Foster existing relationships and help individuals expand their social networks and community involvement
Coordinate and attend medical appointments; support wellness through proper nutrition, physical activity, and adherence to healthcare recommendations
Collaborate with clinicians, therapists, family members, and other team members to ensure holistic, individualized care
Serve as an advocate and leader in supporting individuals and families in achieving goals and navigating services
Accurately document data and maintain confidential records in accordance with agency protocols and regulatory standards (e.g., HIPAA)
Train and coach other staff in intervention strategies and accurate data collection as needed
Manage individual funds responsibly, maintain receipts, and assist with personal shopping and budgeting
Maintain clean and organized living environments, including individuals' bedrooms and common areas
Maintain open, professional communication with families, guardians, and care team members
Attend and participate in supervision, staff meetings, and ongoing training for professional development
Promote a positive team culture through collaboration, reliability, and constructive communication
Provide monthly on-call coverage and respond to after-hours incidents as assigned
Follow all Seven Hills Foundation policies, procedures, and safety protocols
Perform other duties as assigned by supervisors or leadership
Qualifications
Education & Experience:
- High school diploma or equivalent required.
- Experience in direct support, disability services, or related areas.
- Prior experience working with individuals with intensive behavioral challenges preferred.
Skills and Knowledge:
- Valid License. Good driving record.
- Varied interests and personal connections in the local community.
- Proven record of conscientious, responsible behavior (work, volunteer or school history.)
- Ability to work as part of a team. Positive, constructive.
- Strong communication skills and cultural competence.
- Relevant computer skills are essential.
