The Care Partner is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our Terrace Club neighborhood based on the Designated Care Partner concept. Responsible for promoting alliance with the residents to help create successful days through prompting, cueing, and reminders. The Care Partner role includes providing hands on care and physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP) while maintaining a safe, comfortable home like environment. Responsible for demonstrating the Mission for Sunrise, “to champion quality of life for all seniors” in accordance with federal, state, and local standards, regulations, and Sunrise policies to promote the highest degree of quality care and services to our residents.RESPONSIBILITIES & QUALIFICATIONS
Job DescriptionEssential Duties
As a part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is a fundamental part of this job. Our foundational belief is the sacred value of human life. The unique responsibilities for this role include but are not limited to the essential functions listed as follows:
Resident Care
- Review, read, notate and initial Daily Log to document and learn about pertinent information and any resident’s physical and behavioral changes.
- Review and provide input to the Individualized Service Plans (ISP) and Leisure Assessment.
- Assist in the Terrace Club Program, based on individual resident interests and skills that provide stimulating programs, according to Terrace Club Guidelines and Policy.
- Partner with residents to help create successful days through prompting and cueing reminders.
- Assist in implementation of the Daily Program Calendar for each day’s events and journal entry with the residents.
- Demonstrate effective use of written communication skills to assists residents with daily journal entries.
- Observe, report, and document symptoms and conditions of residents.
- Review designated assignments for the day.
- Responsible for their designated group of residents during the shift, know where their residents are and physically check on them throughout the shift.
- Attend Cross Over Meeting
- Notify supervisor and Resident Care Director if a resident has increased care needs.
- Check residents for changes in skin condition, side effects of medications, and changes in overall health conditions.
- Inform supervisor of any resident changes of condition and resident family concerns.
- Respond to security system quickly and appropriately and answer all call bells immediately and take appropriate action including resetting call bells.
- Answer, direct, and/or respond to all phone calls in a timely, courteous, and professional manner.
- Toilet all residents as indicated and dispose of all incontinence products properly to ensure sanitation of resident suite and community restrooms (universal care precautions).
- Communicate with families and is a resource as a Designated Care Partner.
- Help maintain the self-respect, personal dignity, and physical safety of each resident adhering to the standards of Resident Rights.
- Maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment.
- Ensure the established safety regulations are followed at all times.
- Maintain universal care precautions of cleanliness, hygiene, and health standards.
Risk Management and General Safety
- Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations.
- Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, MSDS (Material Safety Data Sheets), and Lockout Tagout procedures.
- Report all accidents/incidents immediately.
- Report all unsafe and hazardous conditions/equipment immediately.
- Ensure any cords, carts, equipment, and other hazards are kept out of the way at all times, not blocking exits, and in compliance with fire codes.
- Comply with all infection control techniques, placement of bio-hazard containers, and removal techniques, procedures, and policies.
- Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
- Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately.
- Ensure oxygen tanks are stored safely, exchange guest/residents tanks when empty, and monitor to make sure liters of oxygen are at prescribed levels.
Housekeeping & Laundry Services
- Maintain and clean resident’s room as assigned including making beds, emptying trash, putting clothes away appropriately, spot sweeping, and mopping as needed.
- Wash resident’s laundry as noted in the ISP and as needed; encourage resident participation.
- Dispose of all incontinent products properly using universal care precautions of cleanliness and promote sanitation.
- Wash and fold dining room linens and napkins.
- Complete assigned housekeeping tasks.
- Maintain common areas clean and tidy at all times.
Dining Services
- Serve meals in the dining room and work in the dining room as assigned.
- Promote and ensure a pleasant dining experience during all meals.
- Assist with dining room set up and clean up as assigned.
- Participate in pre-meal meetings.
- Follow residents diets as indicated on ISP.
- Observe, note, chart, and report residents changes in eating habits.
- Respond to the dining needs of the residents and guests while maintaining Sunrise hospitality and service standards.
- Complete meal tracking log as designated.
- Provide room service delivery as needed.
- Practice safe food handling at all times to prevent an outbreak of food borne illness in compliance with universal care precautions.
- Encourage resident involvement with all phases of meal times.
Training, and Contributing to Team Success
- Participate as a member of a team and committed to working toward team goals.
- Demonstrate in daily interactions with others, our Team Member Credo.
- Commit to serving our residents and guests through our Principles of Service.
- Contribute in the overall engagement programs and processes (customer and team member engagement) including participating in the team member engagement survey and engagement improvement planning workshops.
- Attend regular meetings; Town Hall, Department Team Meetings, Cross Over, and others as directed by the Supervisor/ Department Coordinator.
- Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.

Experience and Qualifications
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed throughout this job description are representative of the knowledge, skills, and abilities required.
- High School diploma / GED accepted and may be required per state/provincial regulations
- Must be at least 18 years of age
- Previous experience working with seniors preferred and desire to serve and care for seniors
- Ability to make choices, decisions, and act in the resident’s best interest
- Ability to react and remain calm in difficult situations
- Possess written and verbal skills for effective communication and a level of understanding
- Competent in organizational and time management skills
- Demonstrates good judgment, problem solving, and decision making skills
- As applicable, all Sunrise team members who drive a Sunrise vehicle must sign the Driver job description and understand the key essential duties for safety and regulatory compliance
