The acute Rehab Admissions & Discharge Social Worker is responsible for performing social work assessments, interventions as needed, and planning for patients discharge needs. Job Requirements
Has excellent understanding of rehab admission criteria, both pre-admission, admission and discharge and insurance payor. Requirements for rehab admission. Measurement: Chart Review. Obtains prompt authorization from payors prior to patient admission to the Rehabilitation Center. Collaborates effectively with payors and provides timely documentation for admission authorization review. Measurement: Chart Review.
